To mail a donation, print the form below and mail it to the address provided. Contributors are gratefully recognized in our Annual Report as follows:

o President's Council: $1000 and over

o Patrons: $500 - 999

o Benefactors: $250 - 499

o Subscribers: $100 - 249

o Sponsors: $50 - 99

All gifts, large and small, make a real difference!

Would you like to honor or pay tribute to someone in a very meaningful way? This could be for a special anniversary or birthday, recognition for an accomplishment or just to say thanks. Give a tribute or honor gift to the Association. Your honoree will receive a card stating what you have done. You will be doing two important things - recognizing the recipient and furthering our services to those who are blind so they can live independent lives.
